Vermont uses BFIS for licensing records and CDDIS for financial-assistance reporting. Home census and background-check paperwork support home registration; health forms come from the Department of Health.

Check the form and your provider type.
An official agency form is issued or listed by the agency; that label alone does not establish mandatory use. A required state form is identified as mandatory in the cited instructions. A required record identifies information you must maintain; its format may be flexible. An optional template is a suggested format. Official instructions / portal links explain the application or record process.
Read “Applies to” and “Format and applicability” for each item. Home, group-home, center, local licensing, food-program, and subsidy requirements can differ.

Official instructions / portal
BFIS Licensing Applications and Reapplications
Manage licensing applications, reapplications, variances, incident submissions, and staff associations.
Applies to
Licensees and directors, including family child care homes.
Who completes it
Applicant, licensee, or director
Where it goes
Through the program's Bright Futures Information System account.
When to use or update
For licensing transactions; the public guidance does not establish a universal reapplication deadline.
Format and applicability: Public instructions lead to an account portal. The page returned 403 when opened; indexed official text was available.
Identify every home resident, with the applicant listed first.
Applies to
Family child care home applicants.
Who completes it
Home applicant
Where it goes
Keep a copy; send to Child Development Division through its current submission instructions or secure email.
When to use or update
During home application; confirm how to report later household changes with the licensing technician.
Format and applicability: Official one-page PDF, revised August 2022. Its older fax instruction should not be treated as current record-check submission guidance.
Current Record Check Authorization — Digital Form Guidance
Authorize and submit a child care background check using the current digital or PDF process.
Applies to
People subject to CDD child care background checks; single-site and multisite programs.
Who completes it
Person undergoing the check, with program information
Where it goes
Use the current CDD forms page's digital submission or current PDF instructions.
When to use or update
When a record check is requested. Forms dated June 5, 2025 or earlier stopped being accepted March 16, 2026; fax submissions ended after March 20, 2026.
Format and applicability: Announcement returned 403 on open; current indexed agency text was available. Obtain the current version rather than the older 2023 PDF still found online.
Selected official forms and public instructions were reviewed on September 14, 2026. This is a practical starting list, not a complete certification of every record, retention period, local rule, or individual application.
Several current DCF pages returned HTTP 403. Their indexed official text supported the portal and sample entries; the individual home census and Health Department sources opened successfully.
An older accessible DCF directory pointed to a 2023 record-check PDF. It is excluded because the March 2026 announcement retires forms dated June 5, 2025 or earlier.
The registered-home startup page describes a preliminary application, census, record checks, and a home visit, but was also inaccessible on open. Confirm first-application steps with the Child Care Business Technician.
Sample admission and drill layouts are not represented as compulsory state formats.
Use the agency’s current original and revision instructions. Keep confidential child and staff records in your own secure record system.
Use this sheet to check which items apply to your program. A listed item or a checked box does not establish that your program meets licensing requirements.
